Sample Preview Pages
Below are samples of the forms output by the program. These are ‘preview’ screens, which is to say they contain the information that will be printed out when you elect to do so. You should first review the screens to make certain information is stated as you want it. If not, you can cancel (close) the preview, return to any screen, and edit as needed.
The first several screens shows you how to select the Print/Save/Preview options. The drop-down menus at the top of the screens are shown also.
After you click on the thumbnail below, if the form does not fill the screen, click on the expansion icon which will appear in the lower right for a larger view.
